Hospital to Home gets animated

Since July 2013, Iriss's Innovation and Improvement team has been leading a project to design a care pathway to support the transition from hospital to home for older people.

The first stage of this project (July 2013-April 2014) involved gathering reflections from practitioners based nationally in Scotland on what the pathway looks like for them and the associated problems. This information was collated into a visual pathway map.

Case study

This case has been designed to familiarise students with the framework, key principles and statutes surrounding social work intervention with vulnerable adults. It consists of a three stage scenario describing the difficulties in the lives of an older couple, and their family, as they become increasingly dependent on community, residential and hospital-based services to protect them from danger and to promote their welfare.
