This multi-agency chronology template was produced by a working group linked closely with the National Group on Chronologies. Although designed for use with children and young people, its structure can be adapted for use in Adult Support and Protection contexts too.

Financial harm affecting older adults: Information and comms pack

This comms and information pack accompanies the theme of ASPire 2025, financial harm affecting older adults. It links with the video designed to raise awareness of the issue. The main comms pack contains ideas for social media posts related to financial harm, useful numbers for the public, images that can be used on local information and Zoom / Teams background templates.

Pathway for capacity assessments for protection-based decisions (Grampian): Screening tool

This tool provides a structured way for professionals to consider systematically whether an adult requires a formal assessment of their capacity for decision making. It is designed to support professionals to consider key factors relating to an adult’s capacity at an early point and record that information. This document is featured in the Grampian pathway for capacity assessments for protection-based decisions.

This pathway is designed to clarify the process for seeking a capacity assessment where there are protection-based decisions linked to concerns about mental incapacity. Included with the pathway are guidance notes, information on escalation arrangements, and appendices that provide suggested text to request capacity assessments.

The hoarding support checklist

This is a one-page tool designed to help professionals stay on track when supporting adults with a hoarding disorder (or who display hoarding behaviour). It is a checklist that helps remind professionals of key strategies in their response. These include a trauma-informed approach, working in a multi-agency way, and building relationships. The tool is produced by Clouds End, a UK-wide social enterprise supporting positive and supportive strategies to address hoarding behaviour.

Decision-specific screening tool (Renfrewshire)

This tool can be used where there is doubt about the adult's capacity to make and/or implement a specific decision. It aims to assist a practitioner consider the various elements involved in the decision-making process. It is not suitable for medical or complex decisions.

Working together to improve adult protection: Risk assessment and protection plan (formats and explanatory notes)

This resource, from the Adult Protection Shared Learning Initiative, presents a set of formats and standards intended to be used as part of a broader assessment and care planning process. The document begins with explanatory notes concerning those formats, which were developed from detailed work involving social work, health, police services and voluntary organisations. All adult care groups (older people, physical disability, learning disability, mental health, and substance use) are represented in the project work, which also drew on child protection and criminal justice experience.

Chronologies: templates and guides (Pan Lothian Partnership)

A chronology is a record of significant events, recorded in the order they occur. Events may be either positive or negative, but they reveal patterns of behaviour, and their impact on an individual’s life.
