Working together to improve adult protection: Risk assessment and protection plan (formats and explanatory notes)

This resource, from the Adult Protection Shared Learning Initiative, presents a set of formats and standards intended to be used as part of a broader assessment and care planning process. The document begins with explanatory notes concerning those formats, which were developed from detailed work involving social work, health, police services and voluntary organisations. All adult care groups (older people, physical disability, learning disability, mental health, and substance use) are represented in the project work, which also drew on child protection and criminal justice experience.

Chronologies: templates and guides (Pan Lothian Partnership)

A chronology is a record of significant events, recorded in the order they occur. Events may be either positive or negative, but they reveal patterns of behaviour, and their impact on an individual’s life. This suite of resources, developed by the Pan Lothian Partnership, offers templates and guides to creating effective chronologies.

Learning Review toolkit (Aberdeen)

A Learning Review is a multi-agency review process. It aims to reflect, increase understanding and identify key learning in Adult Support and Protection. In order to support effective Learning Reviews, Aberdeen Adult Protection Committee have created this suite of resources. They are designed to help all those involved in conducting Learning Reviews.

Form: Recording initial Adult Support and Protection inquiries and investigations (Angus)

This form is used in Angus for recording initial inquiries and investigations. It makes clear the delineation between initial adult protection inquiries and full adult protection investigations. It is used by all council officers in Angus. Includes a template to record chronologies of significant events. Use of this form has contributed to positive inspection results in Angus.
