The recently published National Health and Social Care Workforce Plan (Part 2) calls for the need to progress and co-produce social care and multi-disciplinary workforce planning tools that support the delivery of high quality care. This is care that reflects the new health and social care standards and enables service redesign and new models of care. The need for tools to support partnership working is one area of focus.

It aims to provide a framework for navigating conversations that can support an effective partnership.
